APPOINTMENT PROCEDURE

At Mainstream Physical Therapy, our knowledgeable, professional, and courteous

staff are here to assist you with scheduling and your treatment plan.

l Appointments are made promptly, and can be the same, or the next day upon physician request.

l Mainstream Physical Therapy accepts most insurance plans and is Medicare certified.

l We offer convenient early and late hours and are open Monday through Friday.

l Saturday hours are available December through April.

You have a choice in who provides your Physical Therapy and Rehabilitation Services.

Let your doctor know that you'd like to receive your treatment from Mainstream Physical Therapy.

Tell your doctor that you would like to go to Mainstream Physical Therapy for your treatment.

Site Visits are not common, but are Available Upon Request, as needed and required in the treatment plan.
